The Coalition to Transform Advanced Care (C-TAC) partnered with Discern Health (Discern) to support its continued efforts to build the Advanced Care Transformation Index (ACT Index), which C-TAC developed to assess the state of advanced illness and end-of-life care in the United States. In this project, Discern conducted research to identify evidence-based best practices that states can implement to improve performance on one of the ACT Index measures, Advance Care Planning (ACP).
