bug: two 'cancel' buttons are displayed in dialog when user is prompted to update running workspace to latest template version

Reported by customer on ticket 4700.

Reproduced using:

Coder extension version: 1.11.3
IDE: Cursor (reported by customer), VS Code (Version: 1.106.2)

To reproduce:

1. Start a Coder workspace.
2. Authenticate the extension to the Coder deployment
3. Push an update to the template to the Coder deployment
4. Open/Connect to the running workspace via the extension
5. Receive prompt asking to update to the latest version which shows two Cancel buttons.
