COVID-19: In partnership with community sponsors and health plans, Wider Circle offers its unique neighborhood care programs to more than 320 communities nationwide, including specialized initiatives for vulnerable populations in isolation via virtual programming and peer-to-peer community support.

Last week at the Florida Association of Health Plans meeting, I had the opportunity to share a little bit about the work Wider Circle is doing alongside Dr Debra Jones of Vheda Health and Beth Kidder, MPP of Health Management Associates. Our discussion focused on lessons learned through efforts to support and improve maternal health among some of our most vulnerable communities. A few notables: -Importance of human connection and support. While technology will help and augment connection, it doesn’t replace the trust built through a conversation — be it with a care provider or someone who is living your shared experience. -Trust is built in small ways. And takes time. For some of these communities, there is deep mistrust and we must recognize that in our engagement model. -Urban and rural communities both face barriers and challenges with access, and for underserved communities, care deserts prevent even the most willing from getting the care they need. Our work needs to continue and strong public and private commitments through pathways like the Rural Health Transformation Program are vital to sustain and scale these efforts. #maternalhealth #medicaid

At Wider Circle, we believe second chances create ripple effects. Kevin's journey proves it. Three years ago, Kevin came home from incarceration facing diabetes, obesity at 315 pounds, and overwhelming barriers. When Community Engagement Specialist Celina Jolivette reached out, she connected Kevin to nutritious food, healthcare navigation, and most importantly—community where he didn't feel alone. The transformation was remarkable. Kevin lost 100 pounds, got his diabetes under control, and started the Justice Reform Initiative.
