Alert: The numbers do not reflect the reality: Medication error statistics reported by national and international agencies represent only a small fraction of the true harm caused by unsafe medication practices worldwide. In the United States, the Food and Drug Administration receives over 100,000 medication error reports each year, yet this number grossly underestimates the true prevalence of harm: research indicates that more than 7 million Americans experience some form of medication-related injury annually, with studies estimating only about 1-2% of actual incidents are reported to authorities. This dramatic underreporting mirrors global patterns, where millions of incidents every year go undocumented due to barriers such as stigma, poor infrastructure, and inconsistent surveillance systems. Underreporting limits the effectiveness of international and national patient safety initiatives. Across settings—from hospitals to home care—errors can occur during prescribing, dispensing, or administration, with many stakeholders remaining unaware or reluctant to report incidents. In some regions, well over 90% of medication errors are never formally captured in official statistics—this figure applies not only to the United States but also to Europe, Asia, and Africa, according to World Health Organization studies. Systemic barriers such as insufficient education, fear of repercussions, and lack of standardized reporting mechanisms mean that true rates of harm remain invisible in both developed and developing countries. Consequently, published figures cannot be relied upon to reflect the actual magnitude of medication-related harm. The gap between documented and real incidents highlights the urgent need for improved reporting systems, greater transparency, and widespread preventive strategies. International leadership, such as WHO’s “Medication Without Harm” initiative, targets a 50% reduction in severe medication harm over five years, a goal only attainable through global cooperation, robust data collection, and active engagement from providers, patients, and policymakers worldwide.
