How to Sign Up for MyChart
You must be 18 or older to create a MyChart account. To sign up for a MyChart account, go to UVMHealth.org/MyChartSignUp. You will need to complete two-factor authentication to verify your contact information before the signup process can be completed. After verifying your contact information, you will then be prompted to create a username and password for MyChart.
If you would like to sign up for a MyChart account after receiving a billing statement or a printed After Visit Summary, go to UVMHealth.org/MyChartSignUp and click "Sign Up Now.” Enter the activation code printed at the bottom of your document.
You can also request access to MyChart in person, during a provider visit, or request an emailed link to sign up.

No, your activation code is not your user ID or password. You will use this code only once to log into MyChart for the first time. The code will expire after you have used it or after 45 days.
When you log into MyChart for the first time, you will be asked to create your own unique MyChart username and password.
To update personal information, such as your home address, email or preferred name and pronouns, click on “Personal Information” under the Account Settings section of the main navigation menu.
