Chronic care was never truly designed for continuity. It has long operated around scheduled appointments, paperwork, and episodic check-ins. But behaviour change doesn’t happen every 90 days, symptoms don’t wait to change until the next follow-up, and motivation doesn’t align with clinical calendars. Health manifests in everyday decisions, barriers, fluctuations, and routines outside the clinic. What patients actually need is support woven into daily life: 1. When habits slip: Someone (or something) that nudges them back on track before a small lapse turns into a setback. 2. When symptoms change: Real-time interpretation and management, not a waiting period until the next appointment. 3. When life gets in the way: Reminders, encouragement, and clarity that meet them where they are in terms of motivation. This is where AI can finally bridge the gap in chronic care. The value lies not in replacing clinicians, but in extending their reach beyond limited touch points. AI can continuously monitor patterns and behaviours, surface insights that clinicians can trust, and personalise interventions based on behaviour science and real-world context. With the right application of AI and technology, chronic care delivery can evolve toward: 1. Continuous monitoring and timely alerts. 2. Personalised nudges and behaviour-support strategies. 3. Real-time interpretation of symptoms and adherence trends. 4. Clinician-ready insights rather than raw unfiltered data. 5. A care experience that feels present, responsive, and adaptive. Where do you see AI adding the most value across the chronic care continuum?

This World Diabetes Day’s theme, “Diabetes & Well-Being” reminds us that good health is never just clinical. Its emotional, behavioral, and deeply human. And that’s exactly where the real work in diabetes care happens. A tech-savvy user base is typically associated with metros, but recently, we shared an important piece of research at the International Diabetes Federation Congress: Tier 2 cities outperformed metros in diabetes outcomes, and nearly 65% of users reduced their HbA1c by over 1%. It’s a simple insight with a powerful message. Intuitive digital healthcare, when combined with empathetic handholding, can drive meaningful change anywhere. At Lillia, AI doesn’t replace our care team. It amplifies their ability to support people. For elderly users and those in underserved geographies, our coaches become trusted partners, making digital health feel accessible rather than intimidating. They understand the hesitation, frustration, or self-doubt that can come with learning something new, and meet those moments with patience and reassurance - whether it’s adopting a new nutrition plan, learning to track meals, or staying consistent with medications. Our two independent American Diabetes Association-published studies¹² reaffirm the same pattern: when people receive timely guidance, empathetic coaching, and data-driven insights, glucose control improves, medications reduce, and health outcomes transform. Technology improves access, but it’s the human connection that protects well-being and sustains long-term change. On this World Diabetes Day, we’re celebrating the people, tools, and care models that make diabetes management not just possible but doable. To healthcare leaders and providers: How are you weaving empathy into your digital health journey? 12,000+ attendees. 900 sessions. 1 clear message from #HLTH2025: The future of chronic care won’t just be AI-driven, it’ll be human-led and trust-built. Across thousands of conversations, we heard the same refrain: - Clinicians don’t need more dashboards, just want fewer clicks - Payers don’t need innovation that promises ROI but what proves it - Everyone wants AI that acts responsibly, not just predicts - HIPAA, FDA alignment, and explainable AI aren’t optional but are the foundation of trust - Clean, contextual, ethically managed data is the new currency of efficiency and care HLTH 2025 validated what we believe and are building at Lillia - healthcare solutions that are AI-powered, clinically proven, and deeply human.
