    Independent primary care is disappearing. Out of 174,561 physicians, between age 31–40, more than 80% now work in corporate-owned practices. Young doctors aren’t choosing this. They’re entering a system where independence no longer feels like an option. And when physicians lose autonomy, patients lose something too, real relationships, time, trust, and transparency. They become a number and not a name. We can’t keep pretending consolidation is harmless. It has to change because it's changing the ❤️ of healthcare. Independent physicians built American medicine.   • Patrick Nelli highlights a critical challenge we solve for self-insured employers. High-Deductible Health Plans were designed to control costs, but instead created barriers to preventative care resulting in unknown out-of-pocket costs and higher downstream costs such as emergency visits and specialty care. Our marketplace makes it possible for any self-insured employer to offer DPC benefits at $0 cost-sharing for employees on HDHPs regardless of the employer's size or geographic distribution.

    "I have health insurance, but I can't afford to use it."   This contradiction defines millions of Americans on High-Deductible Health Plans (HDHPs). With median deductibles of $2,750 representing over one-third of the median American's total savings account balance, many employees avoid care due to cost uncertainty and expensive out-of-pocket bills.   Under new policy regulations, employers can now offer Direct Primary Care memberships with $0 cost-sharing for HDHP participants. This means unlimited primary care visits with no deductible applied, prescription medications at wholesale prices, and 24/7 access to their clinician, all without the fear of surprise bills.   But here's what really matters: DPCs excel at catching problems early when they're easier and cheaper to treat. This preventive approach delivers real results: 41% fewer emergency department visits, 22% fewer specialist referrals, and 13% reduction in total healthcare costs.   The math is simple: A $75/month DPC membership that prevents just one emergency room visit ($2,000) pays for itself many times over.   The biggest barrier to healthcare access isn't insurance coverage, it's cost uncertainty.     I recently came across a great story from Paul Thomas, M.D. at Plum Health DPC, and it’s a perfect example of what makes Direct Primary Care different.
