Through Medical Home Network’s partnership with Koda Health, patients facing serious illness are getting the guidance and reassurance they need to make informed care decisions.   One patient with stage 4 lung cancer and COPD had hesitated to complete her planning documents. With Koda’s support, she met with a patient advocate to share her concerns, ask questions, and explore her options.   By the end, she felt relief knowing she wasn’t facing these choices alone: "So many of us put off completing the paperwork. Health centers meet patients where they are, often serving communities with complex needs and limited access to clear, culturally relevant information. When providers help patients understand their health, patients are empowered to make informed decisions, better manage chronic conditions, and build trusted relationships with their care teams.
