Through Medical Home Network’s partnership with Koda Health, patients facing serious illness are getting the guidance and reassurance they need to make informed care decisions. One patient with stage 4 lung cancer and COPD had hesitated to complete her planning documents. With Koda’s support, she met with a patient advocate to share her concerns, ask questions, and explore her options. By the end, she felt relief knowing she wasn’t facing these choices alone: "So many of us put off completing the paperwork. This kind of support means everything."
