MyChart makes managing your health care, medical records, test results and communication with providers easy for you and your family. Learn more about setting up proxy access for your loved one. 

How to Setup Proxy Access

Spouses, partners, caregivers, parents and legal guardians may need to access another patient's MyChart account. To make sure all patient information is private and protected, they must request proxy access. A proxy is a person who has been granted access to another patient’s MyChart account or medical information.

Requesting Proxy Access

To request proxy access to another patient's account or medical information, you must have your own MyChart account. You will need to submit an authorization form based on the age of the patient.

Obtaining Information for Patients Under 12

If you are seeking proxy access for a child under 12, you must be the child’s parent or legal guardian (or have permission from the child’s parent or legal guardian). To request access, complete and submit the MyChart Proxy Access Request & Authorization form for a child under 12 years old. You can do this through MyChart or download and complete the form.

You can return the form to the location where your child receives care or submit the form by mail.

If your child is born at a UVM Health hospital, proxy access is automatically activated for parents.

Obtaining Information for Patients 12 to 17

Parents or legal guardians who manage the medical care of a patient aged 12 to 17 can request access to their online medical record. 

To request access, download and complete the MyChart Proxy Access Request & Authorization form for a child between 12 and 17 years old. To grant full proxy access, the patient must also sign this form. You can return the form to the location where your child receives care or submit the form by mail.

Please note that online form submission is not available for patients aged 12 to 17.

Obtaining Information for Patients 18 and Older

Adults who manage the medical care of another person who is 18 or older can request access to the patient's online medical record. The patient or their legal representative must give their consent.

To request access, download and complete the MyChart Proxy Access Request & Authorization form for an adult 18 or older. The patient or their legal representative must also sign this form. You can return the form to the location where the patient receives care or submit the form by mail.

Please note that online form submission is not available for patients over the age of 18.

Patients 18 and older can invite others to review their chart. You must be a current UVM Health patient to access the Proxy Invite feature.

Submitting a Proxy Application Form

Completed forms may be mailed to the Health Information Management office at the health care partner where the patient receives their care.

If the patient is receiving care at UVM Medical Center, you may also email completed forms to MyChartProxy@UVMHealth.org. You will receive a confirmation email once the proxy application is processed. We will contact you regarding any questions or issues.

Proxy Access FAQs

What information can be seen with proxy access?

Proxies can view any medical information available in MyChart. This includes upcoming visit details, After Visit Summaries, visit notes, health summaries, current medications, health alerts and reminders, test results, and care team messages.

If you are a proxy for a patient aged 12 to 17, they may choose to limit the information you can access.

How long does proxy last?

It depends on the age of the patient.

  • Patients under 12: Proxy access lasts until the patient turns 12, at which time it will automatically convert to default access. Patients may choose to grant full access. Proxies will be notified prior to this date.
  • Patients 12 to 17: Proxy access lasts until the patient turns 18 or revokes access.
  • Patients 18 and older: Proxy access lasts until the patient revokes access.
How can I remove someone's proxy access to my account?

You can remove someone’s access to your medical information in two ways.

  • You can call your doctor’s office and ask them to remove a person’s proxy access.
  • You can remove a person’s access to your account on the Friends and Family Access page in MyChart.
Why can't I share a MyChart account with my spouse or child, or use the same email address for all acounts in my family?

The privacy of each individual’s personal health information is essential. MyChart protects information from unauthorized access by giving each person their own account. This means each person needs their own email address and password. Please do not share your username or password with others.

Can I ask questions regarding a family member from my MyChart account?

We strongly discourage this. Communication about another patient would be placed in your health record, not the patient’s, which could potentially jeopardize medical care. If you have proxy access to another patient’s account, all communication about them needs to be through their own MyChart account.

What happens to proxy access when a patient passes away?

When a patient dies, their MyChart account is automatically deactivated. Existing proxies are granted an additional 30-day extension to their proxy access of the deceased patient.
